Documentation required by Reading Dental Sedation Clinic

If you are wanting IV sedation whilst undergoing dental treatment, please read the patient pre-appointment information below. For more information on IV sedation, please click here

We have also included information for dentists wishing to refer patients to our clinic.

Pre-appointment information for patients undergoing IV sedation:

• All patients must be accompanied by a responsible adult and escorts must remain on the premises at all time whilst treatment is in progress
• You will not be allowed to go home by public transport (bus or train). Your escort must drive you home or accompany you in a taxi, which can be arranged (taxi fare at own cost)
• Patients must not EAT, DRINK or SMOKE for 4 hours prior to treatment. The only exception to this rule is for patients on medication. Please take all medicines as normal with a small amount of water. Please do not starve for prolonged periods, because it may make you feel unwell. Diabetic patients MUST have a light meal 2hrs prior to treatment
• Please download and read all the documents in detail and sign where necessary
• Remember to bring all the documents with you on the day of treatment
